to use SparsePak on WIYN in "Shared Use" mode on ___[specific
allocated dates, or, number of proposed nights during specific
observing semester]___ for our research project entitled:

We have collectively agreed to this request consistent with the
policy, use, and publication conditions described in the memo
http://www.astro.wisc.edu/~mab/research/sparsepak/shared_use_memo,
dated 30 Nov 2001.

This agreement is independent of any TAC process or approval of
telescope time for the above project, and is specifically and only for
the time period listed above.
